Job Overview
We are seeking a compassionate and organized Dialysis Care Coordinator to manage and support patient care for individuals undergoing dialysis treatment. In this role, you will coordinate care plans, monitor patient progress, and collaborate with healthcare teams to ensure optimal outcomes.
Key Responsibilities
Coordinate dialysis treatment schedules and patient care plans
Monitor patient progress and report any changes to clinical staff
Collaborate with nephrologists, nurses, and care teams
Educate patients and families on dialysis procedures and care plans
Ensure accurate documentation of patient records and treatment updates
Assist with patient intake, assessments, and follow-up care
Support compliance with healthcare regulations and safety standards
Coordinate with insurance providers and assist with billing processes
Maintain communication between patients and healthcare providers
Ensure a positive and supportive patient experience
Required Qualifications
Bachelor’s degree in Healthcare Administration, Nursing, or related field (or equivalent experience)
2–4 years of experience in dialysis care, patient coordination, or healthcare support
Strong knowledge of dialysis procedures and patient care practices
Excellent communication and organizational skills
Ability to manage multiple patients and schedules efficiently
Familiarity with healthcare documentation and record-keeping
Preferred Qualifications
Certification in dialysis technology or renal care (e.g., CCHT or similar)
Experience in dialysis centers or nephrology clinics
Knowledge of insurance verification and healthcare billing
Familiarity with electronic health record (EHR) systems
Understanding of patient safety and infection control practices
