Job Overview
We are seeking a detail-oriented Patient Care Documentation Specialist to ensure the accuracy, completeness, and compliance of patient records. In this role, you will collaborate with healthcare providers to maintain high-quality documentation that supports patient care, coding accuracy, and regulatory requirements.
Key Responsibilities
Review patient records for accuracy, completeness, and consistency
Ensure proper documentation aligns with clinical care and coding standards
Collaborate with healthcare providers to clarify and improve documentation
Identify gaps or discrepancies in patient records and recommend corrections
Maintain and update electronic health records (EHR/EMR)
Ensure compliance with HIPAA and healthcare regulations
Support audits and quality assurance initiatives
Track and report documentation trends and improvement opportunities
Assist in training staff on documentation best practices
Required Qualifications
Bachelor’s degree in Health Information Management, Healthcare Administration, or related field
1–3 years of experience in healthcare documentation, medical records, or clinical support
Knowledge of medical terminology and clinical documentation standards
Familiarity with EHR/EMR systems
Strong attention to detail and organizational skills
Good analytical and communication abilities
Preferred Qualifications
RHIT, RHIA, or related certification
Experience in healthcare compliance or medical coding support
Knowledge of ICD-10 and documentation guidelines
Experience with healthcare audits and quality improvement
Strong problem-solving and process improvement skills
