Job Overview
We are seeking a compassionate and detail-oriented Clinical Intake Nurse to manage the initial patient intake process and ensure accurate clinical documentation. In this role, you will assess patient needs, coordinate care services, and facilitate smooth admission workflows. This position is ideal for individuals interested in patient coordination, clinical assessment, and healthcare operations.
Key Responsibilities
Conduct initial patient assessments and collect medical histories
Review referrals and determine eligibility for services
Coordinate patient admissions and intake processes
Communicate with physicians, case managers, and healthcare teams
Ensure accurate and complete clinical documentation
Verify insurance information and authorization requirements
Educate patients and families about care plans and services
Maintain compliance with healthcare regulations and policies
Track intake data and generate reports as needed
Support care coordination and patient transition processes
Required Qualifications
Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN)
Active Registered Nurse (RN) license in New York
2–4 years of clinical nursing or intake coordination experience
Strong knowledge of patient assessment and care planning
Familiarity with healthcare regulations and documentation standards
Excellent communication and interpersonal skills
Strong organizational and multitasking abilities
Preferred Qualifications
Experience in intake, case management, or care coordination roles
Familiarity with EMR/EHR systems
Knowledge of insurance verification and authorization processes
Certification in case management (e.g., CCM)
Strong problem-solving and patient advocacy skills
