Job Overview
We are seeking a compassionate and organized Rehabilitation Case Nurse (RN) to manage and coordinate patient care in rehabilitation settings. In this role, you will oversee treatment plans, monitor progress, and ensure effective recovery outcomes.
Key Responsibilities
Manage rehabilitation patient cases and care plans
Coordinate with therapists and healthcare teams
Monitor patient progress and adjust care plans
Administer medications and treatments as prescribed
Maintain accurate patient records and documentation
Educate patients and families on recovery plans
Ensure compliance with healthcare standards and protocols
Coordinate discharge planning and follow-ups
Support patient mobility and therapy activities
Ensure patient safety and quality care
Required Qualifications
Bachelor’s degree in Nursing (BSN) or Associate Degree in Nursing (ADN)
1–3 years of experience in rehabilitation or clinical nursing roles
Valid RN license to practice in Indiana
Strong knowledge of rehabilitation and patient care practices
Excellent communication and coordination skills
Ability to manage multiple patient cases
Preferred Qualifications
Experience in rehabilitation or case management roles
Certifications such as BLS, ACLS
Familiarity with electronic health records (EHR) systems
Strong patient coordination and monitoring skills
