Job Overview

We are seeking a detail-oriented and compassionate Clinical Care Coordination Assistant to support care coordination activities. In this role, you will assist healthcare teams in managing patient care plans, scheduling services, and ensuring smooth communication between departments.

Key Responsibilities

Assist in coordinating patient care plans and services
Schedule appointments, referrals, and follow-ups
Maintain accurate patient records and documentation
Communicate with healthcare providers and patients
Support care transitions and discharge planning
Ensure compliance with healthcare policies and procedures
Track patient progress and update care information
Assist in resolving care coordination issues

Required Qualifications

Bachelor’s degree in Healthcare Administration, Nursing, or related field
0–2 years of experience in healthcare coordination or support roles
Strong organizational and communication skills
Basic understanding of healthcare workflows and patient care processes
Good problem-solving and multitasking abilities
Ability to work in a fast-paced healthcare environment

Preferred Qualifications

Experience in hospital or clinical coordination roles
Familiarity with electronic health records (EHR) systems
Knowledge of healthcare compliance (HIPAA)
Understanding of care management and coordination processes
Strong interpersonal and patient support skills
